| Stacy B. Ketcham, FLMI, HIA, Vice President of Strategic Initiatives |

| Ms.
Ketcham, Vice President of Strategic Initiatives, is responsible for project
management of strategic initiatives. Prior to joining IHC in April 2005, she
was Assistant Vice President at World Insurance Company located in Omaha,
Nebraska. During her nearly 15-year career with World she worked in almost
every aspect of the administration of the company including managing twenty
mergers and acquisitions over a ten-year period, coordinating multiple
high-level strategic initiatives, and directing multiple TPA relationships. |

| In 1989 Ms. Ketcham received her Bachelor of Arts, English Literature, from
Washington University, St. Louis, MO, and included extensive math and
engineering coursework. She went on to receive her Master of Arts, English
Literature from Creighton University, Omaha, NE in 1991. While attending
Creighton she taught undergraduate English Literature courses through a
fellowship program. She became a Health Insurance Associate of America (HIA) in
1992 and a Fellow of the Life Management Institute in 1993. In 1999 she
received her Certificate in COBOL and Mainframe Programming from the Creighton
Institute. |

| Born in Omaha, Nebraska, Ms. Ketcham recently relocated to Phoenix. She has
served on the Alumnae Board of Duchesne Academy of the Sacred Heart and the
Alumni and Parents Admissions Program of Washington University in St. Louis. |
