| Daryl J Zee, RHU, Vice President of Product Development and Implementation |

| Daryl Zee is responsible for creating new and innovative health insurance
products, including consumer-driven arrangements, managing the implementation
process and producer training. He also oversees the design of Web-based quoting
and enrollment tools. |

| Mr. Zee joined IHC in January of 2003 and has more than 20 years of insurance
industry experience with emphasis on fully insured products, self-funded health
insurance, Section 125 Plans, Health Savings Accounts and Health Reimbursement
Arrangements. He has played key roles in sales, marketing, administration and
product development for several leading organizations including IDS American
Express and Diversified HealthCare Services, Inc. Prior to joining IHC, Mr. Zee
acquired and successfully managed a small business that employed 25
individuals. |

| Mr. Zee graduated San Francisco State University in 1985 with a Bachelor’s
Degree in Management and Marketing. He earned the Registered Health Underwriter
“RHU” designation and holds an Arizona Life and Health Insurance License. |
