| A 40+ year veteran of the insurance industry, Mr. Costas joined IHC in 1996 and
is a Vice President. He is responsible for performing various functions from
Carrier Relations, Compliance, special Markets, etc.

| Mr. Costas was formerly the President of AEGON USA’s Group Division and managed
all group major medical business distributed through a network of independent
third party administrators. Prior to his nine plus years with AEGON, Mr. Costas
held various posts in health, life and annuity divisions with Bankers Security
Life and College/University Corporation. |
