Frank P. Fugiel, CLU, FLMI, FALU, ALHC, MHP
Senior Vice President, Network and Medical Management Services

| Frank Fugiel is responsible for researching, recommending and monitoring claim
cost containment initiatives, preferred provider networks, and consumer-based
value added product and services. He is also responsible for coordinating
oversight responsibilities for IHC’s non-owned TPA relationships. |

| Mr. Fugiel joined IHC in 1996 as Vice President, Business Development, which
included responsibilities for sales, marketing and all aspects of product
development. In 2006, he was promoted to Senior Vice President, Business
Development. He became Senior Vice President, Provider Network and Medical
Management Services in 2007. |

| Mr. Fugiel has held key executive positions at AEGON’s U.S. Group Health
Division. He has also held key managing and technical positions in operations,
underwriting, claims, and risk management at several other major insurance
companies, including Untied Chamber’s Health, Employers Health Insurance
Company and Prudential Insurance Company.

| Along with his multiple professional industry design nations, he is recognized
as one of the co-founders of the Midwest Anti-Fraud Insurance Association,
which has been instrumental in providing guidance and communication between law
enforcement agencies and life and health insurance companies. |

| A native of Chicago, Illinois, Frank attended the University of Illinois, in
Business Administration and also served in the Marine Corps Fourth Air Wing. |
