IAC, we are here to support you with accurate and timely service. Please

| Coverage & Eligibility / Precertificaton |
| For Precertification and coverage & eligibility verification, please
call the telephone number printed on the back of the insured’s ID Card. |

| Submit Claims: |
| IAC has partnerships with more than 60 PPOs. To submit electronic claims to IAC
you will need to know the PPO (if any) indicated on the front of the patient’s
ID card. |

| PHCS, First Health, CCN, Accountable Health Plans, InterWest, Ohio Health
Choice, Multiplan, ppoNext, and ppoNext-Roswell: Submit your electronic
claims to either ENS or THIN. Our payor id is IAC01 for both networks. Submit
paper claims to the address shown on the card: IAC, PO Box 828, Park Ridge, IL
60068-0828. Should you require further assistance, please call IAC at
602.906.6251. |

| For all other PPOs: If you are a participating provider in the PPO
indicated on the card, follow the instructions you have received from that PPO
to submit electronic claims to them. Your PPO will then forward the claims to
IAC. |

| If there is no PPO Network logo on the front of the card OR if you are not a
participating provider: Submit your electronic claims to either ENS or
THIN with our payor id of IAC01.

| Dental providers: Send paper claims to the address on the card. We do
not accept electronic dental claims. |
